Position Highlights
As a Lead Physician, you will:
- Provide Exceptional Primary Care: Deliver comprehensive, patient-centered care to a panel of members.
- Coach & Mentor Providers: Guide clinicians on delivering primary, urgent, and community care while effectively leading interdisciplinary teams.
- Collaborate with Leadership: Work closely with the Medical Director to provide clinical leadership to physicians, nurses, behavioral health specialists, and medical assistants.
- Drive Market Goals: Play an integral role in achieving quality, utilization, and engagement goals by participating in leadership meetings and representing the organization to community and healthcare partners.
Clinical Care:
- Deliver primary care services to a panel of members in a collaborative, team-based environment.
- Manage acute exacerbations of chronic conditions onsite or determine when a higher level of care is necessary.
- Facilitate same-day/next-day urgent appointments for members recently discharged from hospitals or skilled nursing facilities.
- Develop and implement evidence-based care plans in collaboration with patients, families, and care teams.
- Ensure patients receive robust transitional care throughout their continuum of care, including hospital discharges, emergency room visits, and home care.
Leadership & Mentorship:
- Coach and mentor providers and Advanced Practice Providers (APPs) to deliver exceptional care and achieve quality metrics.
- Facilitate population health huddles and multidisciplinary meetings to guide teams toward achieving clinical and utilization goals.
- Develop clinical protocols to upskill clinical staff and ensure top-of-license practice.
Administrative & Collaborative Duties:
- Partner with the Medical Director to foster relationships with academic programs and community organizations.
- Track and achieve specified quality metrics to ensure optimal outcomes.
- Participate in leadership meetings and serve as an ambassador for the organization to community-based organizations, hospital systems, and payer partners.
Working Conditions
- Operates in a professional office environment with occasional exposure to clinical settings.
- May require wearing protective attire, such as gloves and masks.
- Frequent travel to other centers or corporate offices as needed.
Physical Requirements
- Ability to remain stationary for long periods of time while operating computers and office equipment.
- Must communicate clearly and exchange accurate information with patients, providers, and staff.
Requirements
- Active, unrestricted medical license in the state of employment.
- 5+ years of clinical experience in primary care, community health, or hospital-based care.
- Experience leading interdisciplinary teams in clinical or administrative settings.
- Strong expertise in chronic care management, value-based care models, and population health.
- Proven success in mentorship, team leadership, and healthcare service delivery improvement.
- Exceptional problem-solving, project management, and interpersonal communication skills.
