COMPANY PROFILE:
The hiring organization is an innovative neighborhood health center offering primary care, cardiology, and general wellness services to geriatric patients. They provide a wide range of services to ensure each and every patient receives the highest level of healthcare and attention possible, including a full suite of diagnostics and treatments, nutritionists, acupuncture, telehealth, and much more.

POSITION DESCRIPTION:
The new physician will work in a team-based care environment, in a 100% outpatient and clinic-based capacity. The centers are large, modern spaces with on-site services such as lab, echo, EKG, and much more.

The care teams consist of:

- nurses
- medical assistants
- case managers
- social workers
- community outreach specialists
- and more

The clinic also has contracts with Uber and Lyft to provide transportation for patients who need it.

Physicians hold an average panel size of 400 patients. This model allows physicians to see fewer patients per day (an average of 10 to 13 visits) and spend more time with them, with a “quality over quantity” approach.

Specific duties of the role include, but are not limited to:

- provide assessment, planning, primary care, and continuous medical coverage
- provide direction and support to nurse practitioners and members of the care team
- collaborate on care planning decisions
- assist in coordinating care delivery

The organization uses the Athena EHR and provides ongoing training in value-based care.

REQUIREMENTS:
- Alabama medical license
- DEA
- Board Certified or Eligible, Internal Medicine or Family Medicine
