Details:
- Setting: Primary Care, 3.5 days clinic based, 1.5 days virtual (from home)
- Schedule: Full-time, weekdays only
- Teaching: Optional with faculty appointment
- Phone Call Only: average of 2 call weekends per year and 1-weeknight ~ 2 months
- Access to Behavioral Health, High Risk Nurse Care Managers & Social Workers
