We are seeking Board-Certified/Board Eligible Internal Medicine Physicians to join a forward-thinking, patient-centered practice with openings in Mesa, Phoenix, Glendale, and Chandler, AZ. Job Responsibilities

Deliver comprehensive, evidence-based primary care services to adult patients in an outpatient setting.

Collaborate with cross-functional care management teams to provide integrated, coordinated care that improves patient health outcomes.

Utilize clinical insights and population health tools to guide personalized treatment plans and care delivery.

Work with highly trained support staff to reduce administrative burdens and streamline workflows.

Provide virtual care as needed using advanced telehealth platforms integrated into our EMR system.

Participate in quality improvement initiatives to enhance care delivery, patient satisfaction, and clinical outcomes.

Maintain thorough and compliant documentation within the EMR system.

Qualifications

Board Certified or Board Eligible in Internal Medicine.
2+ years of outpatient clinical experience post-residency preferred.
Active and unrestricted State medical license or eligibility for licensure in Arizona.
Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
Active DEA registration or eligibility to obtain one.
Current or eligible Medicare/Medicaid enrollment.

Skills

Strong interpersonal and communication skills to foster trust-based relationships with patients and care teams.
Proficiency with EMR systems; Athena EMR experience preferred.
Ability to work collaboratively in a multidisciplinary, team-based environment.
Proficiency in using population health tools and clinical data to drive quality improvements and outcomes.
A commitment to providing patient-centered, compassionate care that improves health outcomes and enhances the overall patient experience.
