Job Responsibilities
Collaborate with cross-trained care management teams to provide integrated, coordinated care that improves patient outcomes.
Utilize data-driven tools and clinical insights to develop personalized treatment plans and deliver high-quality care.
Partner with highly trained clinical and operational staff to streamline workflows and reduce administrative burdens.
Provide care virtually as needed using advanced telehealth platforms and optimized EMR technology.
Participate in quality improvement initiatives and population health programs to enhance care delivery and outcomes.
Maintain thorough and accurate documentation in the EMR system to ensure compliance and quality standards.
Qualifications
2+ years of clinical outpatient experience post-residency preferred.
Active and unrestricted State medical license or eligibility for licensure.
Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
Active DEA registration or eligibility to obtain one.
Current or eligible Medicare/Medicaid enrollment.
Skills
Proficiency with EMR systems; Athena EMR experience preferred.
Ability to work collaboratively in a team-based environment to deliver patient-centered, coordinated care.
Proficiency in using clinical data and population health tools to drive quality improvements.
