Among other services, they offer medical care coordination, dental care, vision care, audiology services, pharmacy delivery, in-home primary care, physical therapy, and more.


POSITION DESCRIPTION:
The physician will work within a program of all-inclusive care for the elderly (PACE), which provides comprehensive medical and social services to eligible at-risk participants living in the community. Most participants are dually eligible for Medicare and Medicaid/Medi-Cal.

Within the program, the physician performs in-person comprehensive assessments and coordinates with an interdisciplinary care team to develop individual care plans for participants. Physicians oversee care of participants in facilities and hospitals, including admissions and discharges for acute, short-term, and long-term placement, as well as the use of medical specialists.

Specific duties of the role include, but are not limited to:

- provide assessment, planning, primary care, and continuous medical coverage
- provide direction and support to nurse practitioners and members of the care team
- collaborate with community-based physicians and care teams in discharge planning
- communicate with care teams on changes with program participants
- collaborate on care planning decisions
- assist in coordinating care delivery
- help maintain care of participants in facilities
- make regular facility visits, per patient needs
- participate in rotating night and weekend call coverage

The organization uses an EHR.

Due to the size and growth pattern of the organization, this position will be eligible for advancement opportunities, including leadership roles (if desired).


REQUIREMENTS:
- California medical license
- DEA
- Board Certified, Internal Medicine or Family Medicine
- experience with the geriatric population
