LOCATION:

This practice sees patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), and via limited house calls. They are currently hiring in the Cleveland area and will work with physician candidates during the interview process to ensure a mutually-agreeable geographic territory in which all patients / buildings will be located.

This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of where they call home. They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.
POSITION DESCRIPTION:
The physician will serve as the primary care provider for a dedicated panel of geriatric patients.
All patients are located in facilities or private homes. Depending on the territory, the practice is able to offer different mixes of facilities (ALFs / ILFs / SNFs) and home-based patients. During the initial phone interview, the practice will look at the physician’s home location and discuss this in detail.
In SNF settings (with no travel during the day), physicians see an average of 15 to 16 patients per day.
When traveling between ALFs, ILFs, and homes, physicians see an average of 9 to 12 patients per day (this is adjusted based on actual travel time).
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- evaluate patient conditions and adjust treatment plans as necessary
- perform minor procedures and wound care as required (and as settings permit)
- educate patients and families on health management, disease prevention, and treatments
- work with other providers as needed to coordinate comprehensive care
Physicians serve as the supervising providers for the practice’s nurse practitioners, who require minimal oversight - the practice only hires experienced nurse practitioners.
The practice uses the eClinicalWorks EHR.
On-call is phone triage only with very low volume. Providers rotate the on-call responsibility, with a shift every 10 weeks (or less) for full-time providers. Part-time providers will participate in on-call on a negotiated schedule.
