Annashae is seeking a 40 hour per week Primary Care Provider to provide medical management/services at a mental health center located in Southwestern Illinois, close to the Missouri border.

Hours: Weekdays; Mon - Fri (8:00 am to 5:00 pm provider can work 8 -10 hours per day).

Qualifications:

• Current unrestricted Illinois License
• Board certified/eligible in Internal or Family Medicine
• Current Illinois Controlled Substance certificate
• Current unrestricted DEA certificate
• BLS
• Ability to pass the required drug and finger print screening.

Job Duties Include, but Not Limited to:

• Provide treatment to a caseload of patients on the pavilion
• Evaluate assigned cases at intake
• Sign certificate of admission and testify in court
• Order and interpret lab tests
• Conduct initial physical exams(H&P) and makes diagnoses of patients
