We are looking for a dedicated and passionate Family or Internal Medicine Physician to join our team. You’ll work alongside a comprehensive care team, including a health coach, to provide patient-centered care that typically requires referral to specialists in today’s fragmented healthcare system. As a key physician, you will be committed to delivering outcomes-based care and data-driven population health, striving to restore primary care to its original purpose: serving patients and communities with high-quality, holistic care.

Compensation: $250,000.00 - $300,000.00, Plus potential Sign-On Bonus

Essential Functions:

  • Deliver evidence-based primary care to 10-15 patients daily, focusing on prevention, chronic and acute care management, and disease case management.
  • Collaborate with patients and your care team, including an onsite Whole Health Coach, to create personalized, biopsychosocial care plans that honor each patient's unique needs and diverse background.
  • Provide comprehensive care aligned with the medical home model, including collecting health histories, performing physical exams, conducting diagnostic tests, and prescribing medications with the convenience of onsite labs and pharmacy services.
  • Assess the needs of your patient population and encourage participation in annual health evaluations, health coaching, and wellness promotion activities.
  • Offer clinical leadership through participation in team meetings, clear communication, process improvement, and support for change management efforts.
  • Rotate with other providers in the region to ensure after-hours coverage.
  • Engage in organizational committees (e.g., weekly provider meetings, Clinical Advisory Committee, Quality Improvement Committee) to help optimize processes and programs.

Education & Experience:

  • MD or DO degree.
  • Completed residency in Family or Internal Medicine (Family Medicine preferred).
  • Board Certification by ABMS (ABFM/ABIM) or AOA.
  • 3+ years of primary care experience.
  • Current, unrestricted medical license and valid DEA number.
  • Experience using EMR systems (NextGen preferred).
  • Familiarity with coding and documentation, including Medicare Advantage and HCC.
