Callen-Lorde Community Health Center is dedicated to providing high-quality, sensitive, and non-judgmental healthcare to LGBTQ communities and people living with HIV. We are seeking a dedicated and compassionate Medical Provider to join our team as a Primary Care Provider (PCP). As a PCP at Callen-Lorde, you will play a vital role in delivering comprehensive care, making a positive impact on the lives of our diverse patient population.

As a Primary Care Provider, your responsibilities will include:
- Screening, diagnosing, and treating Callen-Lorde patients.
- Providing care in a sensitive and non-judgmental manner.
- Making appropriate internal and external referrals.
- Offering comprehensive care based on pre-arranged schedules (15-20 min appointments)
- Documenting care promptly and following charting protocols (EHR: EPIC)
- Ensuring appropriate follow-up for ordered care, including referrals and test results.
- Providing care to patients living with HIV and Trans individuals. Medical providers new to HIV and Trans care will participate in didactics, precepting, and case conferencing in order to build competency in HIV and Trans care.
- Contributing to continuous quality improvement efforts.
- Maintaining familiarity with clinical guidelines in various areas of care.
- Functioning effectively as part of a caregiving team.

Qualifications
Current New York State Registration
DEA Controlled Substances Registration Certificate
CPR Certification
Experience serving Callen-Lorde’s target populations preferred
Board certification or eligible in Family Medicine, Internal Medicine or Infectious Disease for MDs/DOs

Commitment to community-based primary care.
Ability to work within a team-based structure in a pod model.
Bilingual (Spanish/English) preferred.
