Lead the Opening of our Brand New Senior Primary Care Office in New Port Richey, Florida.

Conviva Senior Care Centers, a subsidiary of Humana Inc., is seeking a Family, Internal, or Geriatric Medicine Physician for our newest office. Grow with us as we expand our reach to help more Seniors live better, healthier lives.

You have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes.   • Guaranteed base salary + 20% bi-annual quality incentive, total compensation up to $300,000

Job Responsibilities

A day in the life of a Conviva physician:

-Monday-Friday 8:00am-5:00pm; no weekends
-Join an experienced team with two Advanced Practitioners and a Care Team
-See 12-15 patients per day. Opportunity to spend more time with patients
-Longer authorized appointments (45 minutes)
-Acts as a lead and active participant and key source of medical expertise with the Care Team through daily huddles
-Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care

Qualifications

Required Qualifications

-Doctor of Medicine or Osteopathic Medicine
-Board Certified/Board Eligible in Family, Internal, or Geriatric Medicine
-Valid, unrestricted license in the state of desired employment (or willing to obtain)
-Current, unrestricted DEA license

Preferred Qualifications:

-Two to five years directly applicable experience preferred
-Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
-Knowledge of Medicare guidelines and coverage.
-Knowledge of HEDIS quality indicators.

Working Hours

Monday-Friday, 8-5 pm.
Outpatient only, seeing 12-15 patients per day under a Value-Based Care Model.
