COMPANY PROFILE:
The hiring organization is the largest value-based care, senior-focused primary care network in the country. They are a value-based care provider focused on quality (not quantity) of care, with teams consisting of physicians, advanced practice professionals, nurses, medical assistants, and more. Their approach allows them to provide an unmatched experience for the geriatric population, with a model positioned to provide higher quality care and better outcomes, especially in rural communities. They are currently hiring due to growth.

POSITION DESCRIPTION:
The new physician will work as a lead in a team-based care environment, working in a 100% outpatient and clinic-based capacity. This value-based model focuses on quality of care for patients, with teams consisting of:

- physicians
- advanced practice providers (nurse practitioners / physician assistants)
- pharmacists
- care coaches
- nurses
- medical assistants
- behavioral health providers
- referral coordinators
- and more

This model allows physicians to see fewer patients per day and spend more time in visits, with a “quality over quantity” approach.

New patient visits are typically scheduled for 60 minutes and established patient follow-ups for 40 to 45 minutes.

Specific duties of the role include, but are not limited to:

- provide assessment, planning, primary care, and continuous medical coverage
- provide direction and support to nurse practitioners and members of the care team
- collaborate on care planning decisions
- assist in coordinating care delivery

The organization uses an EHR and provides ongoing peer-to-peer collaboration and long-term career cultivation.

REQUIREMENTS:
- Texas medical license
- DEA
- Board Certified or Eligible, Internal Medicine or Family Medicine
- 2+ years experience preferred (not required)
