- Schedule: 3-4 Days Per Week, Monday-Thursday, 8:00a-4:00p
- Job Setting: Outpatient
- Types of Cases: Standard Primary, Acute, and Chronic Care, Adults Only
- Credentialing Timeframe: 2 Weeks
- Must have an active Drug Enforcement Administration (DEA)
- Must have an active Arizona state license
