Details
* Monday - Friday (8a-5p)
* Mix of peds/adults
* Pediatrics preferred (not required)
* No call
* Outpatient
* 18 patients minimum per day
* Chart closure within 72 hours
* Support Staff - 1 RN and 2 MAs
* EPIC EMR
* 30-90 days estimated credentialing timeframe
Requirements
* Board Certified
* California licensure only (no IMLC)
* BLS certification
* Clean History
* Active DEA needed before start
The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Primary series plus booster, regardless of date received or 1 dose of the current COVID vaccine (2023/2024 formulation)
