We provide quality services that are accessible, patient-centered, continuous, and comprehensive. - The only FQHC in Brevard County employs approximately 65 physicians and APPs. - We work with existing providers in our community to coordinate service to our patients. - Following a patient-centered medical home model, with ID department within the confines of primary QUALIFICATIONS: - Lead Physician for Infectious Disease- working alongside APRN and per diem Physician in coordinating care for patients including: HIV, Hepatitis C, Chronic Hepatitis B, STD, and other outpatient acute and chronic diseases - Board Certification Required -Must see adult patients only - Active DEA and BLS - Spanish is a plus - Proficiency with EMR - Must understand 340B, Ryan White, and coordination of HIV services - Must work well with others
