Job Responsibilities:

• Provide high quality patient care and a full scope of primary care services to patients
• Review patient history and perform physical examinations
• Evaluate, diagnose and provide appropriate treatment and patient care
• Provide quality clinical diagnostic skills in the delivery of patient care
• Review all test results and recommend suitable management for the patient
• Prescribe medications and formulate ongoing treatment and disease management plans
• Provide relationship-centered care, taking into account the patient’s psychosocial and physical needs
• Collect and record patient information and maintain complete and accurate medical records
• Communicate effectively with the clinical team to insure total delivery of quality care
  

Job Requirements:

• Doctor of Medicine (MD), Doctor of Osteopathy (DO) or foreign equivalent
• Residency program in Internal Medicine and or Family Medicine
• Board Eligible/Board Certified in Internal Medicine or Family Medicine
• Licensed to practice medicine in New York State license
• Active Drug Enforcement Agency (DEA) registration or be eligible for registration
• Active Medicare/Medicaid Enrollment or be eligible to enroll
• BLS or ACLS preferred
• Both experienced and newly trained providers are encouraged to apply
