Internal Medicine Locum, Arcata, CA– Clinic Only, Starting ASAP
The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), Covid-19 Vaccine Series and 1 Booster

- Includes some Chronic Pain Management
- Chart closure within 72 hours
- Schedule = 18 PPD, minimum see 18 PPD
- Monday-Friday 8-5
- Mix of peds/adults (Will consider IM only)
- A decision is typically made within 7-14 days
- BE will be considered if truly BE (5 years or less since residency)
- Active DEA needed before start
- Credentialing takes 30-90 days (six weeks is common if provider submit app./supporting documents timely)
