- Base Compensation: $281,000
- Arrival Bonus of $50,000 at start date
- Qualifying moving expenses up to $20,000
- 15 days PTO, 8 holidays, plus 5 days CME

- Base Compensation of $228,000 plus Work Relative Value Unit (WRVU)-Based Compensation Model
- WRVUs adjusted to allow four weeks out of office
- $50,000 retention bonus at end of Year Two upon good standing
- $50,000 retention bonus at end of Year Three upon good standing

- $2,500 Continuing Medical Education (CME) per calendar year prorated
- Midlevel supervision per one FTE Midlevel $4,500/year when applicable
- Malpractice 100% covered during employment
- License expense paid in full DEA expense paid in full

- Year 2 – 3 Annual Provider Benefit Stipend
- Cell phone stipend $75/month, starting year 2
