We are seeking a compassionate and skilled Geriatric Medicine Physician to join our team in West Springfield, MA. The ideal candidate will have a passion for working with elderly patients and providing high-quality, comprehensive care tailored to the unique needs of older adults. Key Responsibilities:

Provide primary and specialty care to geriatric patients, including those with chronic conditions, cognitive impairments, and multiple comorbidities.
Conduct comprehensive assessments of patients, including medical histories, physical exams, and functional assessments.
Diagnose, treat, and manage common geriatric conditions such as dementia, frailty, osteoporosis, cardiovascular diseases, diabetes, and more.
Develop personalized care plans that address the physical, emotional, and social aspects of aging.
Coordinate with multidisciplinary teams, including nurses, social workers, physical therapists, and other specialists, to ensure comprehensive care for patients.
Educate patients and families on managing age-related conditions, preventive care, and health promotion strategies.
Stay current with best practices in geriatrics and evidence-based medicine.
Participate in community outreach programs, health education initiatives, and advocacy for the elderly population.
Document patient interactions, diagnoses, treatments, and follow-ups in compliance with healthcare regulations and standards.

Qualifications

Board Certified in Internal Medicine or Family Medicine - REQUIRED
Board Certified in Geriatric Medicine - PREFERRED
MA license (or willing to obtain) - REQUIRED
CDS Certification & DEA Verbal Confirmation Registration (prior to start) - REQUIRED
Minimum 1 year working with the frail or elderly population - REQUIRED
BLS (prior to start) - REQUIRED
