CenterWell Senior Primary Care is seeking FM/IM Physicans who have a passion for primary care and want to make positive changes in the lives of our patients and the healthcare industry as a whole.
We’ve had an outstanding year of membership growth which is now expected to be around 825,000 (vs. 775,000). That’s a growth rate of 18% (and 40% of industry growth). While there’s always room to improve our processes and systems, our industry leadership in quality is a standout, helping put us in a position of strength as we begin the 2024 selling season this October. We also continue to advance our CenterWell capabilities from Primary Care with over 300 senior-focused centers now operational, serving 300,000 patients; to Home Health in evolving our value-based care model and improving quality, with our Star ratings for more branches at 4.5 star or above, taking us from 18% in January 2022 to 50% today; to Pharmacy with continued strong performance in meeting the needs of our customers, including the significant influx of new Medicare Advantage members. And now adding Cardiology Specialty in our Centers.

That's why we limit our physicians' patient panels focusing on quality over quantity seeing 12 to 15 patients daily (650 panel) with a Care Team that includes Advanced Practice Providers, Pharmacist, Social Workers, Behavioral Health Specialist, Registered Nurses, Care Coaches, Certified Medical Assistants, Referral Specialists, Certified Coders, and experienced Administrative staff. Qualifications
• Licensure requirements of the state of jurisdiction and ability to maintain licensure
• Graduate of accredited MD or DO program of accredited University
• Board Certification in Family Medicine, Internal Medicine or Geriatric Medicine with continued certification throughout employment
Job Related Experience
• Minimum of two to five years directly applicable experience preferred
• Experience managing Medicare Advantage patients with understanding of Best Practice in a coordinated care/value based relationship environment
• Knowledge of Medicare guidelines and coverage • Knowledge of HEDIS quality indicators
