- Remote Opportunity: Primarily this position is remote from the providers home. Once a week may be expected to conduct team meetings from an office space and possibly see patients in the patients home once in a while.
- The Medical Director will not have a patient panel, but is expected to support coverage and ad hoc visits as needed. Schedules will vary.

- BC/ Internal Medicine or Family Medicine or Geriatric Medicine
- 3+ years of experience in outpatient practice preferred
- Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but NOT required
- Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues
- Experience using a metrics-driven approach to the provision of medical care and/or quality
projects
- Experience successfully driving teams towards achievement of metrics
- A passion for mentorship and team development
- A flexible and positive attitude, including being comfortable with ambiguity

consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC)
- Patient panel of about 75 patients, seen monthly
- The Medical Director leads the market clinical teams in providing the best patient care and a
great place to work. Success isnt completing the visit or hitting an R.V.U. target. Success is
high quality care: (1) appropriate hospital admissions, (2) excellent preventive care/chronic disease management, and (3) a great patient experience, which is helping our patients be happy, healthy, and home.
- Together with the Practice Manager dyad partner, the Medical Director embraces these goals tightly, envisions the means to achieve them, and leads the market in pursuit of these ends. The Medical Director has mission-critical leadership responsibilities, including setting an example for how thoughtful in-home primary care is practiced, serving as a formal mentor, coach, and advisor to market providers, and leading critical program-wide strategic initiatives.
