Primary Care Physician

The Position: The Physician participates as a member of the PACE Interdisciplinary Team in the planning, implementation and evaluation of care plans that meet the objectives, standards and policies of the PACE model of care. This Physician demonstrates proficiency in collaborating with the Nurse Practitioners at the site to provide complete primary care to the participants. The Physician is the leader of the Clinical Team whose primary mission is to assist frail elders to remain living in the community for as long as they may do so safely.

Requirements:

  • Board Certified in Internal Medicine or Family Practice, or Eligibility for Board Certification (for recent graduates)
  • Bilingual in Spanish Preferred
  • Board Certified in Geriatric Medicine, Preferred, But Not Required
  • Current License to Practice Medicine in Massachusetts
  • Current Massachusetts DEA Prescription Privileges
  • Current CPR Certification
  • COVID Vaccination Required

Responsibilities:

  • Monday to Friday (8am to 4pm)
  • Provide Direct Patient Care in PACE Programs
  • Oversee Care & Treatment Rendered by PACE Teams
  • Collaborate as Part of an Interdisciplinary Team
  • Carry Out Supervisory Responsibilities
  • On Call Schedule PHONE ONLY 1:15 Weeks
  • 10-15 Patients Per Day
