Responsibilities Patient Care: Round on patients 1-2 days a week in skilled nursing and rehab facilities, providing comprehensive care (H&P, progress notes, admissions, discharges, follow-up visits). Collaboration: Work closely with interdisciplinary teams, facility leadership, and other healthcare professionals to ensure optimal care and outcomes. Care Transitions: Help coordinate patient transfers to appropriate care settings based on their needs upon discharge (home, independent living, assisted living, or memory care). Clinical Leadership: Foster a culture of care excellence and provide leadership within the facility, improving patient outcomes and enhancing the quality of life for older adults. Qualifications Board Certified or Board Eligible: In Family Medicine (FM) or Internal Medicine (IM). Passion for Geriatrics: A dedication to serving older adults and improving their quality of life. Leadership Skills: Desire to play a leadership role within a post-acute care setting. Experience in Post-Acute Care: Preferred, but not required. Lake Havasu City is located along the shores of Lake Havasu, where the city gets it's name from. The area hosts a wide array of events from fishing tournaments, to boating events and even the International World Jet Ski Races. In Lake Havasu, you can visit a piece of history from Across the Pond - London Bridge. The bridge was purchased and brought to the area directly from London. If outdoor recreation is what you enjoy, hike Crack in the Mountain Trail, head to some of the surrounding area national parks, and so much more!
