Post Acute Care Medical Director opening in Albuquerque, NM Join a dynamic team of passionate healthcare professionals dedicated to providing high-quality care to patients in skilled nursing and rehab facilities. As a Physician with us, you ll collaborate closely with a multidisciplinary care team, including nurse practitioners, physician assistants, and pharmacists. Responsibilities * Conduct federally mandated visits, including initial History & Physical (H&P), Medicare-required visits, and acute and follow-up visits, in collaboration with nurse practitioners and physician assistants. * Partner with the interdisciplinary care team, including pharmacists, to optimize medication management and patient care. * Develop and implement individualized care plans based on patient needs and goals. * Complete progress notes using templated formats to efficiently manage patient documentation and support work-life balance. * Lead initiatives in medical directorship roles to improve patient outcomes and team collaboration. Qualifications * Board Certified/Board Eligible in Family Medicine (FM) or Internal Medicine (IM). * Passion for serving older adults and improving quality of life for patients in skilled nursing and rehab facilities. * Ability to work collaboratively within an interdisciplinary team. * Strong communication skills and a desire to lead and mentor clinical staff.
