Part-time Post Acute Care Opening in North Carolina Located in Fayetteville, NC - Raleigh 65m We are seeking a skilled and compassionate physician to join our team in Fayetteville, North Carolina, providing in-person coverage in skilled nursing facilities one day per week. As a part-time physician, you ll work with a dynamic team to manage and improve the care of older adults in a collaborative, patient-focused setting.

  • Customizable one or two days per week schedule
  • Uncapped earning with bi-weekly bonuses

  • Expected compensation is estimated salary range of $72k to $102k with no cap on productivity income potential. Income includes monthly leadership stipend and expectation of 20 patients per day.

Responsibilities Clinical Oversight & Mentorship: Provide guidance and support to clinicians, sharing tools, guidelines, and best practices to ensure quality care. Case Reviews & Quality Improvement: Lead clinical case reviews and participate in care coordination initiatives to improve patient outcomes and reduce hospital re-admissions. Relationship Building: Serve as a liaison between to the community, establishing and maintaining strong relationships with facility leaders, staff, and local healthcare providers. Care Planning & Communication: Develop and implement individualized care plans, collaborate with interdisciplinary teams, and ensure clear communication about patient care and services. Medication Management: Work alongside pharmacists on medication reviews and management to optimize patient care. Visit Requirements: Complete Medicare-required visits, acute and follow-up visits, and facilitate discharge planning when necessary. Transitional Care Recommendations: Provide recommendations on transitional care, durable medical equipment (DME), home health services, or outpatient services after short stays. Quality & Risk Management: Assist leadership in quality improvement initiatives and risk management strategies. Required: Reside within a reasonable daily commute to Fayetteville, NC Clinical Leadership Experience Relevant clinical experience treating patients within the last 12 months CV must contain beginning and ending mm/yyyy format for all work history and education Active / Unrestricted NC Medical License, DEA, Medicare, and Medicaid provider numbers Explanation of any/all time gaps in education and work history since completion of Bachelor Program Located in central North Carolina, Fayetteville is a place nestled among alluring gardens and local city parks.
