**Post-Acute Care Medical Director Opening in North Carolina** Located in Burlington, NC - Greensboro 22m, Raleigh 58m We are seeking a Medical Director to provide clinical oversight, mentorship, and leadership to our clinicians in the greater Burlington, North Carolina area. In this role, you will drive quality care initiatives, mentor clinicians, and collaborate with leadership to ensure the best possible care for our patients in post-acute settings. Key Responsibilities: - Provide oversight and mentorship to clinicians in the local area. - Conduct clinical case reviews and share tools, information, and guidelines for quality care. - Build and maintain relationships within the community and with facility leaders. - Collaborate with leadership on care coordination initiatives and quality improvement efforts. - Develop and implement individualized care plans based on patients' goals. - Oversee Medicare-required visits, acute care, follow-ups, and discharge visits. - Work with pharmacists on medication management and recommendations for patient care. - Advise on transitional care, DME, home health services, and outpatient services post-discharge. - Support risk management initiatives and ensure timely completion of charts and pharmacy recommendations. - Lead and participate in quality improvement projects and care coordination initiatives. Qualifications: - Active/Unrestricted NC Medical License, DEA, Medicare, and Medicaid provider numbers - Clinical Leadership Experience - Licensed physician with experience in family medicine, geriatrics, internal medicine, post-acute care, home health, hospice, palliative care, or hospital medicine. - Proven leadership ability in a clinical setting, with a focus on mentorship and team development. - Strong communication skills and the ability to collaborate with interdisciplinary teams. - Knowledge of Medicare and care coordination guidelines, including quality improvement processes. - Ability to manage a flexible schedule while maintaining high standards of patient care. - Relevant clinical experience treating patients within the last 12 months
