The Hospitalist provides medical care for patients; and facilitates transition of care at discharge from the hospital by communicating a summary of the hospital course, discharge medications, pending diagnostic studies, and follow-up instructions to the primary care provider and pertinent consultants. The Hospitalist assumes responsibility for comprehensive patient care to assure that the health care needs of patients are being met, including attendance at Interdisciplinary or nursing rounds and daily communication with the team caring for the patient; renders quality medical services according to qualifications as an internal medicine or family practice physician with appropriate referrals, follow-up and hospitalization; supports hospital medical staff and other area physicians utilizing the Hospitalist service by overseeing the inpatient experience, including providing follow-up care for patients in the outpatient realm; communicates health status and treatment plan to patient, family and hospital staff; documents information completely for patient care and billing purposes; assists in the review of work flow, office practices and objectives; and teaches medical students, residents and others in graduate health education programs.
Qualifications

- Communicates health status and treatment plan to patient, family and hospital staff
- Documents information completely for patient care and billing purposes
- Conducts educational activities in required curriculums as assigned
- Teaches medical students, residents and others in graduate health education programs
- Attendance at interdisciplinary or nursing rounds.
- Daily communication with the team caring for the patient
- Keeps abreast of the latest developments in field, both academically and through applied skill.
- Development of personal and professional knowledge and competence through continuing medical education including maintaining board certification Attends and actively contributes to Meetings
