Requirements:

• Board Certified in Family Medicine – REQUIRED
• Active IA license or IMLC must be in hand at time of submission – REQUIRED
• BLS ACLS ATLS NRP PALS – REQUIRED
• Hospitalist Coverage – REQUIRED
• Provider must be Local (within driving distance to the facility) – REQUIRED
