Position Highlights:
Responsibilities:
- Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
- Drive high-quality, safe, and cost-effective patient care
- Collaborate with interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Provide oversight, guidance, and mentorship to clinicians within facilities
- Schedule: Monday through Friday
- Patient Volume: 15 - 20 patients per day
- Compensation: our compensation is a productivity-based model (fee for service) that offers you control to maximize your income potential; expected compensation is estimated salary range of $171,000 to $277,000 annually
- This full-time role is benefit eligible
- Preferred physicians with a background in internal medicine (IM) or family medicine (FM)
