Position Highlights:
- A diverse patient population: working with a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or in need of rehabilitation offers valuable experience in treating a range of medical conditions.
- An interdisciplinary collaboration: collaborate with a multidisciplinary team, including nurses, therapists, social workers, and other healthcare professionals and enhance patient care by incorporating different perspectives and expertise.
- Long-term relationships: building rapport over time can lead to better continuity of care, improved patient outcomes, and a sense of fulfillment in providing ongoing support to individuals in need.
- Perform primary care in a post-acute setting for short-term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
- New grads accepted
- Post-acute care experience a plus, but not required
- Active and unrestricted Washington license required
- Active and unrestricted Washington DEA required
