Join our post-acute care team to provide care to patients/residents in need of rehabilitative care in our skilled nursing facility.

Responsibilities:
  • Provide oversight and mentorship to our clinicians in the greater Columbia, South Carolina, area
  • Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
  • Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
  • Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
  • Collaborate with leadership to review care coordination initiatives
  • Develop intervention plans, and share ideas about network management issue
  • Work alongside pharmacists on medication management
  • Complete Medicare required visits
  • Complete acute, follow up, and discharge visits as necessary
  • Implement an individualized plan of care for patients' goals
  • Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
  • Facilitate and assist leadership in quality improvement initiatives and risk management
  • Timely response in chart completions and pharmacy recommendations

Experience as a medical director in a skilled nursing facility preferred. However, clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family medicine (FM), primary, internal medicine (IM), adult, post-acute, home health, hospice, palliative and hospital medicine (HM).

Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $177,000 to $225,00 annually with no cap on productivity income potential. Click apply to learn more!
