Responsibilities:
- Hospitalist, primarily day shift, to join an established hospitalist group at a northern NH Critical Access Hospital.
- 5-bed adult ICU and stepdown unit type patients.
- Provide care independently or with consultation. Intubate patients, and manage mechanical ventilators.
- Possess BLS and ACLS for care of adult patients; NRP and PALS for care of newborn and pediatric patients, and run codes together with ED physician on duty.
- Ideally, if appropriately trained and credentialed, provides care for pediatrics, including newborns and attending selected deliveries to care for newborn (e.g., high risk vaginal delivery due to fetal distress or meconium; Cesarean section).
- After normal business hours (during assigned shift), provides phone coverage for community’s primary care practice (Coos County Family Health Services), including calls from patients/families, 2 local nursing homes (CCFHS serves as attending for all such residents), home health agencies, and pharmacies.
- EMR is Meditech Expanse.
- Staffing, schedule, and clinical workload:
- 1 day hospitalist 7 am – 7 pm
- 1 night hospitalist 7 pm – 7 am
- 1 additional rounder, typically for 2-4 hours per morning, depending on census: may be Hospitalist Program Medical Director or hospitalist departing from night shift (especially weekends and holidays)
- Clinical schedule: 24 weeks/year; generally 1 week on/1 week off with 2 weeks of paid vacation
- Monday through Sunday
- Need is primarily day shift; may occasionally be schedule to cover nights.
- Call requirements: None
Typical workload for day hospitalist:
- 2 to 4 admissions or consults
- Rounds on, discharge, or transfer 6 to 12 patients
- Total patient encounters 8 to 16
- May supervise stress tests during selected day shifts (training provided if necessary)
Typical workload for night hospitalist:
- 2-4 admissions or consults
- Assess 1-2 patients at bedside due to instability or change in clinical condition
Best Fit Candidates/Expectations:
- Must be comfortable with rural nature of practice and limited number of specialists on call.
- Minimum of 2 or 3 years of post-residency practice preferred. Selected new graduates will be considered.
- H1-B and J-1 visa candidates will be considered if excellent communication skills and 2+ years experience
Required:
- Endotracheal intubation
- Ventilator management (initial, maintenance, withdrawal)
- Use of BiPAP and CPAP
- Manage codes via ACLS protocols
Optional:
- Arterial puncture for blood gases
- Arthrocentesis and joint injections
- Central venous catheter insertion and management
- Lumbar puncture
- Paracentesis
- Thoracentesis
- Point-of-care ultrasound
Quality/citizenship bonus:
- Up to $15,000 per fiscal year, awarded in November. Depending on performance, each hospitalist could achieve $0; $15,000; or a proportional award in between.
- 50% of such bonus will be based on hospitalist team goals/ accomplishments, and 50% will be based on individual hospitalist goals/accomplishment.

- CME funds: $3,500 For license, DEA, Medical Staff dues, professional memberships, tuition and travel for conferences, etc.
- Commencement bonus: $25,000, Paid on first pay date following hire (up to 17 days). Not a signing bonus
- Relocation: $10,000 to $20,000, Varies depending on distance of move, size of family, etc. Taxable.
- Housing stipend: Potentially, Individually negotiated. Taxable.
- Educational loan reimbursement: Potentially. Individually negotiated. Taxable.

- Plans include a 403(b) qualified plan and a 457(b) non-qualified plan.
- Sick time and extended illness: Sick time at 4 days per year. 1 day = 12 hours. Extended illness bank allocation for new hires is 15 days.
