St. Peter's Health Partners is looking for Hospice Physician
Practice Setting: Hospice Home Care
Reason for Coverage: Retirement and Home Care
Dates of Coverage: ASAP until indefinitely
Call Schedule: Call is on average 1 weeknight per week from 4pm - 8am and 1 in 4/5 weekends from 4pm on Friday until 8am on Monday
Call Ratio: 1 in 4/5
Average Number of Patients Per Day: Variable. The majority of the hospice physician's time is spent in interdisciplinary team meetings, reviewing cases for hospice eligibility, and providing guidance and recommendations to the nurses in the field. Home visits for face to face encounters and symptom management are scheduled as needed in accordance with the hospice physician's schedule on average 0-2 per day.
Documentation System/EMR: EPIC - Comfort/Hospice Application
Provider Must be local - only mileage is reimbursed Submission Requirements
- Board Certified - Internal Medicine / Family Medicine / Emergency Medicine
- Hospice and Palliative Medicine or Geriatrics fellowship training is a plus but not required.
- NY State License
- BLS
- Provider must be local
