- Patient Population: Average of 2-4 patients per day, located in Spartanburg, Gaffney & Union Counties
- Number of Needed Shifts: 10


Job Responsibilities

-Being on call for the hospice house nurses 8a-5p
-1:5 night and weekend call for same as above, calls from IDG team, hospice home nurses, etc
-Manage home patient teams, this would involve home visits, placing orders (using EPIC EMR), refilling medications as needed for patient care while working with the IDG (interdisciplinary group) team that includes Nurse, Aid, Chaplain, and Social Work.
-New patient on team would need a home visit within 2 weeks of admission. (This averages between 5-10 new patients per week).
-Some patients need face to face visits these are short visits of established hospice patients, NP's do the bulk of these, physicians are occasionally asked to do them. This could be anywhere between 1-4 a week.
-Ability and comfort prescribing controlled substances for home patients.

Qualifications

Physician must be Board Certified or Board Eligible
Active SC State License

Working Hours

Schedule: Full-time, 8-5 Monday to Friday with 1:5 night and weekend call
