**Hospice and Palliative Care Physician opening in OH**
**Located in Findlay, 40m to Toledo**
**Full Time Position**
Our Medical Group has a Hospice and Palliative Medicine Opportunity in Northwest Ohio.
-Bridge Hospice & Palliative Medicine Service
-Inpatient and community-based palliative medicine
-Two inpatient hospice care centers, Cerner EMR
-Available by phone or answering service on a 24-hour basis as an on-call consultant to clinicians within Bridge Home Health and Hospice
-Expertise in symptom management of terminal illness
-Lead provider based palliative medicine team in clinic, community and inpatient settings
-Positive service-oriented interpersonal and communication skills
-Knowledge and skills to mentor Hospice team in providing consistent quality patient care
-Interact with the Hospice team for patient care planning
-Participate in agency and community-based programs to educate medical professionals and lay persons in hospice and palliative medicine philosophy and services
-Assist with the development, maintenance and review of patient care policies and procedures
