- Coverage Needed:
- December 2, 2024 - ongoing
- Monday-Friday with on call rotation
- 7:30a-4:30p
- On call approx every 4th weekend/3 nights per month
- Clinical Information:
- Required duties
- Provide Hospice Physician services in keeping with Hospice Regulatory requirements
- Provide support for interdisciplinary team excellence in clinical care, collaboration and respect
- Serve as hospice physician for designated Home Hospice teams
- Attend and participate in their hospice teams’ biweekly IDG meetings.
- Review hospice eligibility for referrals, complete eligibility notes, review hospice admissions with admitting RNs at time of admission, review medications for hospice coverage/noncoverage, take calls from their teams’ nurses regarding symptom management, medication orders, etc
- Serve as hospice attending for patients on their teams who choose hospice as attending
- Round on inpatient hospice services in both hospice inpatient units and acute care hospitals (includes in-person and telehealth rounding)

- Required duties
- Required:
- DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
- Board Certified in Hospice Palliative Care
- Active PA License
