Job Responsibilities
Develop and implement individualized care plans.
Manage symptoms and provide emotional support to patients and families.
Coordinate care with other healthcare providers.
Educate patients and families about the disease process and treatment options.
Participate in multidisciplinary team meetings.
Conduct patient assessments and evaluations.
Document patient care and progress accurately.
Participate in research and quality improvement initiatives.
Provide end-of-life care and support.
Advocate for patient needs and preferences.
Ensure compliance with healthcare regulations and standards.
Maintain up-to-date knowledge of palliative care practices.
Collaborate with social workers, nurses, and other healthcare professionals.
Offer grief counseling and support to families.
Develop and deliver educational programs for staff and community.
Participate in continuing education and professional development.
Manage and mitigate pain and other distressing symptoms.
Support patients in making informed decisions about their care.
Promote a culture of compassion and respect within the healthcare team.
Qualifications
Board certification in Palliative Care
Strong understanding of palliative care principles and practices.
Working Hours
