**Requirements:**
* Previous Hospice and/or Palliative Care experience or completion of fellowship in Hospice and Palliative
* Graduate of an accredited School of Medicine
* Board certified in Internal Medicine, Family Medicine or related Specialty
* Current, unrestricted medical license in Commonwealth of Pennsylvania
* Must be in good standing with the Pennsylvania Board of Medicine
* Certification by the American Academy of Hospice & Palliative Medicine ***preferred***
