Requirements:
  • Board Certification: Board Certified
  • License Required: AZ or IMLC
  • DEA: required.
  • Certification: ACLS and BLS
  • Minimum Experience Required: 3 years.
  • Credentialing Timeframe: 30 days
  • Pay Rate: $470/hr
