- Number of Physicians in Practice: 1
- Hem/Onc only practice
- Adults only
- No Radiation Oncology on-site, but connection to Rad Onc team which is 5 miles from practice
- EMR System: Epic
- Practice Setting: Outpatient
- Average Number of Patients Per Day: 16 - 20
- Trauma Level II
- Board Certified – Required
- Active IL License – Required
