All Residencies & Fellowships must be ACGME Approved

Clean NPDB
Three professional references that have worked with the provider within 18 months
Active MI license (IMLC accepted), DEA, and Cs3
Case/patient logs 24 Hematology patients within 12 months
Board Certified or Board Eligible within 5 years
MyMichigan Medical Center Midland
Hematology/Oncology Outpatient Clinic: Monday-Friday, 8-4:30
Call Required – about 2-5 calls/night
Volume of cases – Approximately 15 visits/day
Outpatient visits consist of new patient referrals, established patients actively in treatment and follow-up appointments
No surgeries or procedures required
BLS required
24 patients with illness related to the practice of Hematology within 12 months
