Coverage Details:
* Recommend plan for 8-hour day
* 3 days/week - Clinic days 8-5pm, Procedure days 7:30 or 8am - 4 or 5pm (2 procedures days and 1 clinic day)
Requirements:
* Board Certified
* Active Indiana license required (no IMLC)
* ABLS, BLS certification
