Seeking a BE/BC Family Medicine doctor to join FQHC facility in VT.

- 4-day work week (32h scheduled hours of patient care), all outpatient, practices cover appointments and walk-ins, attractive telephone only after-hours call schedule (approximately 2-3 call days monthly).

The medical center is a not-for-profit, community-based health care system that serves southeastern VT and southwestern NH. Patient services and resources we offer include but are not limited to integrated Behavioral Health, Lifestyle Medicine, Rapid Access through Behavioral Health, Diabetes Education, Consultative Registered Dietitians, Care Coordination, and a Community Health Team that works to assist with housing and food insecurities, transportation barriers, as well as overall access to care barriers. Additionally our organization works collaboratively with Springfield Hospital to meet the health needs of our communities with additional referral specialists, ancillary support, Inpatient and Emergency Department access.
