The Skilled Nursing facility program needs compassionate physicians to join the team. You will be asked to provide primary care services to SNF residents and care management at 6 local community facilities. You will directly oversee medication management, reconciliation, and medication order placement for patients assigned to their service (patient census). In addition you will complete H&P's, Admission and Discharge orders, and participate in Quality Improvement Committee activities as well as provide SNF Staff Education to ensure care plans are sustained. The ideal candidate will be Internal Medicine or Family Practice trained and preferably experienced in SNF's. This position provides a generous salary of $250,000 depending upon experience.
