Shonto, AZ: OP clinic M-F, no call, Credentialing 5-6 weeks, min 13 weeks, BC/BE, any license

Salome, AZ: OP, 4 x 10, tele call, Credentialing 3-4 weeks, min 6 months, BC/BE, AZ license (Client will wait for provider to obtain)

Maricopa, AZ: OP clinic, M-F, no call, THIS IS A PERM ROLE, credentialing 4 weeks, BC/BE, AZ license (Client will wait for provider to obtain)

Casa Grande, AZ: OP clinic, M-F, no call, THIS IS A PERM ROLE, credentialing 4 weeks, BC/BE, AZ license (Client will wait for provider to obtain)
