Job Responsibilities and Duties:
• Provide comprehensive outpatient primary care services
• Manage chronic care conditions for a diverse patient panel
• Conduct routine check-ups and preventive care visits
• Diagnose and treat acute and chronic illnesses
• Develop and implement treatment plans for patients
• Collaborate with other healthcare professionals in a multi-specialty setting
• Maintain accurate and detailed medical records using Epic EMR
• Participate in care coordination and patient education
• Adhere to FQHC guidelines and best practices
• Complete administrative tasks and documentation as required
