Primary Care MD

Specialty accepted: Internal Medicine or Geriatric Physician
Start date: As soon as credentialed – Ongoing (Minimum 6 month commitment)
Schedule: Monday–Friday, 8am–5pm
Practice setting: Home Health
Patient load: 3–6 patients per day (depending on proximity)
Ages: Adult patients
Procedures required: Primary Care, Wellness Exams, Chronic Disease Management, Population Health, Kidney Care
Requirements: Board Certification, DEA, BLS, Active AZ License
Other: Mileage reimbursement at $0.44/mile; in-person orientation in Brentwood, TN; provider must supervise APPs; strong preference for bilingual providers; opportunity to transition to perm; long-term need with extensive national coverage footprint.
