A facility located in Flandreau, South Dakota is seeking a Family Medicine Physician to join our Providers, including one Family Medicine Physician and three Nurse Practitioners. The new Health Center was built in 2017 and provides patients with full medical home model services including urgent care, imagining and onsite CT, full pharmacy, laboratory, case management, optical, dental, behavioral health and an experienced clinical team. This is a unique and rewarding opportunity to work with an experienced team and quality resources to serve and improve the health status of the community.
Job Responsibilities
Outpatient care and medical home model
High quality services and equipment
Full-spectrum Family Medicine- 40% elderly population
Average 10-12 patients per day.
Continuity of care- build long lasting relationships with IHS eligible patients and their families who live in the community.
Electronic Health Record (RPMS) is planned to be replaced in 2025.
Qualifications
Well trained, and familiar with ambulatory outpatient setting- chronic disease management (diabetes, heart disease, and chronic pain).
Mission driven individual who truly values compassionate care
Team oriented
Doctor of Medicine or Doctor of Osteopathic Medicine with Board Certification (or Board Eligible within 5 years) in Family Medicine or Internal Medicine.
Working Hours
Fridays 8a.m.-4:30pm
No weekends and no call
