Family Medicine Opening in Nashville, TN -Full Time & Permanent Position. Description -Conduct thorough evaluations and assessments of patients, including taking medical histories, performing physical examinations, and ordering diagnostic tests. -Develop and implement individualized treatment plans based on patient needs, medical history, and diagnostic results. -Provide preventive care services, including routine check-ups, immunizations, and health screenings. -Manage chronic conditions such as diabetes, hypertension, and asthma, and provide ongoing monitoring and treatment. -Address acute medical issues, including infections, injuries, and urgent care needs. -Educate patients and their families about health maintenance, disease prevention, and lifestyle modifications. Collaboration and Communication: -Collaborate with other healthcare professionals, including specialists, nurses, and support staff, to provide coordinated and comprehensive care. -Communicate effectively with patients and their families about diagnoses, treatment options, and care plans. -Participate in care coordination and case management to ensure seamless patient care. Clinical Management: -Maintain accurate and up-to-date medical records in accordance with hospital/clinic policies and regulatory requirements. -Participate in quality improvement initiatives and clinical audits to enhance patient care and operational efficiency. -Stay current with medical advancements, best practices, and guidelines in primary care. Qualifications: -Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree. -Training: Completed an accredited residency in Family Medicine, or a related field. -Board Certification: Board-certified or board-eligible in Family Medicine, or a related specialty. -Experience: 2 years of experience in primary care preferred. Exceptional new graduates with outstanding training will also be considered. -Skills: Strong diagnostic and clinical skills in primary care. Proficiency in utilizing electronic health records (EHR) systems. -Learning: Commitment to ongoing learning and professional development.
