Patient-centered full-service Community Health Center supporting diverse social ethnic and economic populations. The Center offers “wrap around” services including dental, behavioral health, primary care, gynecology with on-site pharmacy, and labs.

Job Responsibilities

Work / life balance! Outpatient clinic.
Walk into an established patient panel primarily supporting adults.
* Average daily census: 20-25 patients.

Qualifications

• BE/BC
• Family Medicine or Internal Medicine
• IL license, or eligible
Epic EMR. Spanish language skills highly desirable.

4 day work schedule with 8 hours offered for admin; one weekend shift per month is required. W
